First Eucharist / Faith Formation / & Junior High Applications for 2009 - 2010

PLEASE NOTE: AS OF AUGUST 20, 2009,WE HAVE ADDED TWO ADDITIONAL FIRST EUCHARIST DATES. CLICK ON "FIRST EUCHARIST APPLICATION" BELOW FOR AN UPDATED FORM WHICH LISTS THE ADDITIONAL DATES. IF YOU HAVE ALREADY SENT IN YOUR CHILD'S APPLICATION AND WISH TO CHANGE YOUR FIRST, SECOND & THIRD CHOICE, PLEASE EMAIL THERESE AT tgagliardotto@churchofpresentation.org NO LATER THAN FRIDAY, SEPTEMBER 11, 2009, WHICH IS THE FIRST EUCHARIST REGISTRATION DEADLINE.

First Eucharist Application: This is for children entering second grade this September 2009 and who have also completed one year of faith formation (either here at Presentation or elsewhere). If the child will be attending Catholic school for 2nd grade, this is the only application needed. If the child will be attending public school, both the First Eucharist application and the Faith Formation Application are required. First Eucharist applications will be accepted beginning July 7, 2009 at 7:00am. Deadline for registering for this year's First Eucharist program is September 11, 2009.

Faith Formation Application: This is for families with children entering grades 1-6 this coming September 2009. Fee is $200 per family. Late fee of $225 if returned after July 31, 2009. Please return application and fee to the Faith Formation Office.

Junior High Program Application: This is for families with children entering grades 7 &/or 8 this coming September 2009. If there are no other children in grades 1-6, this is the only application needed. Fee is $200 per family. Late fee of $225 if returned after July 31, 2009. Fee is waived if family is registering other children in grades 1-6 in the Faith Formation program. Please return application and fee to the Faith Formation Office.

Children in grades 9-12 will be receiving correspondence from our Youth Ministry program.
